| - L’efficacité de la thrombectomie mécanique, réalisée dans le cadre des accidents cérébraux ischémiques (AIC), est en cours d’évaluation. Les résultats sont encourageants, notamment sur le plan angiographique, mais les patients ne semblent pas tous bénéficier de la prise en charge sur le plan fonctionnel de façon équivalente. Nous recherchons des paramètres cliniques simples qui pourraient permettre de prédire une évolution fonctionnelle favorable à 3 mois d’une thrombectomie mécanique. Nous avons inclus de façon prospective de mars 2011 à mars 2014, 53 patients admis à l’UNV du CHRU de Tours pour un AIC par occlusion des vaisseaux de gros calibre et pour lesquels l’indication d’une thrombectomie avait été posée. Nous avons analysé la corrélation entre l’évolution et le score NIHSS initial, le délai entre le début des symptômes et la recanalisation, l’étiologie de l’AIC, la localisation de l’AIC et l’administration de la thrombolyse IV. En moyenne, l’âge des patients était de 61 ans et le score NIHSS initial était à 17. Soixante-six pourcents des patients avaient reçu la thrombolyse IV, le délai moyen entre le début des symptômes et la recanalisation était de 406 minutes, le taux de recanalisation favorable était de 75,5%. Nous avons observé 13,2% de transformations hémorragiques symptomatiques et 22,6% de décès. L’évolution était favorable pour 39,6% des patients. Le score NIHSS initial était, parmi ceux examinés, le seul paramètre corrélé à l’évolution fonctionnelle à 3 mois en analyse uni et multivariée. Notre étude montre que la gravité clinique semble prédire l’évolution fonctionnelle sans être influencée par les autres paramètres analysés.
- The effectiveness of mechanical thrombectomy, performed as treatment of ischemic strokes (AIC), is currently being evaluated. The results are encouraging, including the angiographic plane, but patients do not appear to benefit functionnally from the care in an equivalent manner. We are looking for simple clinical parameters that could predict a favorable functional outcome at 3 months of the mechanical thrombectomy. We prospectively enrolled from March 2011 to March 2014,53 patients admitted to the stroke unit of the University Hospital of Tours for AIC by occlusion of large vessels for which the indication for thrombectomy was asked. We analyzed the correlation between the outcome and the initial NIHSS score, the time between onset of symptoms and recanalization. the etiology of the AIC, the location ofthe AIC and the 1V thrornbolysis. Average patient age vas 61 years and the initial Nl1-ISS score was 17 Sixty-six percent of patients received IV thrombolysis, the average time between onset of symptoms and recanalization was 406 minutes, the favorable recanalization rate vas 75.5%. We found 13.2% of symptomatic hemorrhagic transformation and 22.6% died. The outcome was favorable in 39.6/o of patients. The initial NIHSS score was among those considered, the only parameter correlated with functional outcome at 3 months in univariate and multivariate analysis. Our study shows that the clinical severity appears to predict functional outcome without being influenced by the other parameters analyzed.
