| - Introduction : l'accès aux soins urgents en moins de 30 minutes pour l'ensemble de la population est un problème de santé publique. Le bassin de santé d'Espalion est un territoire rural de montagne dont la population âgée est entièrement située en \" zone blanche \". L'objectif principal de cette étude est de comparer la codification CCMU par le médecin généraliste exerçant en \" zone blanche \" à celle établie ensuite par le médecin urgentiste à l'hôpital, pour le même patient. Méthode : il s'agit d'une étude prospective descriptive et analytique centrée sur le bassin de santé d'Espalion. Le recueil de données a été réalisé grâce à un questionnaire expliqué et remis aux quinze médecins généralistes ayant participé. Les critères d'inclusion concernent tout patient vu par un médecin généraliste exerçant dans le bassin de santé d'Espalion et ayant été adressé aux urgences. Résultats : il existe une différence significative entre la codification CCMU du médecin généraliste et celle de l'urgentiste pour les patients en CCMU 1 et 2. Les résultats concernant les CCMU 3, 4 et 5 ne sont pas significatifs. Discussion : une des hypothèses évoquées pour expliquer cette différence est l'évolutivité de l'état clinique du patient du fait du délai de prise en charge lié à l'éloignement entre \" zone blanche \" et SAU. Un risque d'aggravation de l'état clinique du patient pourrait justifier la mise en place de dispositifs visant à réduire à 30 minutes le délai d'accès aux soins et inciter les médecins généralistes à y participer.
- Introduction: access to emergency care in less than 30 minutes for the general population is a public health problem. Espalion health pool is a mountainous rural area in which the elderly population resides entirely within the \"white area\". The main objective of this study is to compare CCMU codification between the general practitioner (GP) working in \"white area\" and the emergency doctor at the hospital, for the same patient. Method: this is an analytical, descriptive and prospective study, focusing on the Espalion health pool. Data collection was made through a questionnaire, explicated and given to the fifteen doctors who wanted to take part. Every patient saw by a GP working in Espalion health pool and sent to A&E (Accident and Emergency) has been included. Results: there is a significant difference between the GP's and the A&E doctor's CCMU codification for patients in CCMU 1 and 2. Results concerning CCMU 3, 4 and 5 are not significant. Discussion: one of the mentioned hypothesis to explain this difference is the change in the patient's clinical status as a result of the care time due to the distance between \"white area\" and SAU. An aggravation risk of the patient's clinical status could justify the creation of procedures aiming to reduce the waiting care to 30 minutes and encourage GPs to participate.
