| - The following research was inspired by three major interrogations. Two questions have allowed for a greater understanding of an internal and external environment of a public hospital. The third question underlines problems faced by a public hospital management and seeks an original answer to the following question: how to best manage a health care institution? As a result of cooperation with a health care institution and an in-depth on-site research, we were able to propose an interesting solution to the above problem. Our representation of the organisation, such as a hospital, is based on internal relationships of processes and activities, such as defined by activity based management. This particular representation of a health care institution opened a new sphere of interrogations related to consequences of such an approach. As a result, we have observed at least three elements. The first was a discovery of a match with a coherent system of responsibilities allowing for their simpler and more efficient division. The second was related to a clear relationship between internal policies and the overall organizational strategy. The third was rich enough to allow a continuous reflection on the improvement of organizational functioning. This study of activity management in a hospital environment lead to a model of an integrated control management including both internal dimensions and external pressures necessary for the administration of a public health care institution. In addition, this research gives a new perspective on the activity based management in the service industry in general.
- Le questionnement ayant conduit cette etude tient en trois interrogations. Deux questions ont permis de saisir l'environnement de gestion du producteur de soins, dans ces aspects externes et internes. La troisieme resume la problematique du directeur de structures hospitalieres publiques ou assimilees et cherche a donner une reponse originale au probleme suivant : comment piloter un etablissement de soins ? Grace a une recherche cogeree avec un partenaire hospitalier et une etude approfondie du terrain, nous avons pu proposer une reponse originale a cette question. Notre representation de l'organisation hospitaliere repose sur un schema des rapports internes fondee sur les concepts d'activites et de processus, empruntes a la methode de la gestion a base d'activites. Avec cette proposition de representation de la gestion hospitaliere, nous nous sommes interroges sur les consequences vehiculees par cette approche. Nous avons note a cette occasion au moins trois elements. Le premier a ete de decouvrir la correspondance avec un systeme de responsabilites coherent permettant un partage plus simple et plus efficient des responsabilites. Le second a permis de donner une ouverture claire sur la strategie, a savoir que nous pouvions, mettre en coherence les orientations internes avec la strategie. Le troisieme etait assez riche pour permettre une reflexion continue sur l'amelioration du fonctionnement de l'organisation. Ce travail sur le management des activites en milieu hospitalier propose en final un schema de controle de gestion integre ou l'on retrouve a la fois les dimensions internes et les pressions externes necessaire au pilotage des etablissements de soins participant
