| - Introduction: The increasing number of patients with retinal diseases implies a constant increase in the number of intravitreals injections (IVI). One of its complications: infectious endophthalmitis, can be extremely serious.Objective: To assess the incidence of acute IVI-related endophthalmitis, its risk factors and prognosis factors.Materials and methods: Retrospective monocentric study, including patients managed at the emergency department of the Rothschild Ophthalmological Foundation (FOR) from 2010 to 2018 for acute post-IVI endophthalmitis (anti-vascular endothelial growth factor or corticosteroids). Visual acuity, clinical and therapeutic management parameters were assessed over a 1-year follow-up period.Results: 34 patients were included, 15 of whom were injected at the FOR for a total of 64852 IVTs performed at the FOR over the study period. The incidence of endophthalmitis was 0.023%. Azyter antibiotic prophylaxis was significantly associated with a higher incidence of endophthalmitis (0.05%) compared to prophylactic antiseptic treatment (0.01; p=0.0026). Visual acuity was significantly higher at 1 month in patients receiving intravenous corticosteroids (p=0.022).Conclusion: Post-IVI endophthalmitis remains a rare event with a possibly lower incidence without antibiotic prophylaxis. In the face of its uncertain prognosis, its prevention is essential.
- Introduction : le nombre croissant de patients atteints de pathologies rétiniennes implique une hausse constante du nombre d’injections intravitréennes (IVT). L’une de ses complications : l’endophtalmie infectieuse, peut s’avérer d’une extrême gravité. Objectif : Evaluer l’incidence de survenue de l’endophtalmie aigu[ë] post IVT, ses facteurs de risques et ses facteurs pronostiques. Matériels et méthodes : étude rétrospective monocentrique, incluant les patients pris en charge aux urgences de la Fondation ophtalmologique de Rothschild (FOR) de 2010 à 2018 pour endophtalmie aigüe post IVT (anti « vascular endothelial growth factor » ou corticoïdes). L’acuité visuelle, les différents paramètres cliniques et de prise en charge thérapeutique ont été évalués sur une période de suivi de 1 an. Résultats : 34 patients ont été inclus dont 15 ont été injectés à la FOR pour un total de 64852 IVT réalisées à la FOR sur la période de l’étude. L’incidence de survenue de l’endophtalmie était de 0,023 %. L’antibioprophylaxie par Azyter était significativement associée à une incidence plus élevée d’endophtalmie (0,05%) par rapport à un traitement prophylactique par antiseptiques (0,01 ; p=0,0026). L’acuité visuelle était significativement plus élevée à 1 mois chez les patients ayant reçu une corticothérapie intraveineuse (p=0,022). Conclusion : l’endophtalmie post IVT reste un évènement rare avec une incidence qui s’avère plus faible en l’absence d’antibioprophylaxie. Devant son pronostic incertain, sa prévention est primordiale.
