| - Introduction: osteoarthritis and tendinopathies are pathologies whose prevalence is increasing, are often resistant to conventional treatments and which tend to become chronic.Biotherapies and more particularly platelet-rich plasma (PRP) play an important role in the management of these pathologies. The aim of this study is to assess the pain in the medium and long term according to the VAS and the functional capacity in pathologies of the musculoskeletal system, following a protocol of PRP injection(s). Method: retrospective cross-sectional quantitative study in patients who suffered of a chronic pathologie resistant to medical rehabilitation treatment, received an injection of PRP at the University Hospital of Reunion's functional sports medicine unit with a minimum follow-up of 18 months. Results: 125 patients participated, divided into 5 subgroups made up of : 58 patients with knee osteoarthritis (KO), 37 with Achilles tendinopathy (AT), 19 with plantar fasciitis (PF), 6 with patellar tendinopathy (PT) and 5 with ankle osteoarthritis (AO). A significant decrease in VAS was observed for each pathology (AT -4.7 ; PF -6.1 ; AO -3.2 ; KO -3.72) except for RT. The pain persists in the medium and long term (AT 19.33 + -11.83 months ; PF 18.52 + -11.50 months ; AO 13 + -13.29 months ; KO 11.48 + -10.23 months). Each subgroup shows an increase in functional capacities assessed by the number of hours of physical activity per week (h/wk) (AT +2.48 h/wk; PF +4.3 h/wk; AO + 4.6h/wk; KO +3.28 h/wk) except for RT. Conclusion: our study is in favor of an analgesic effect in the medium and long term in AT, PF, ankle and knee osteoarthritis and appears to increase functional capacities in patients who suffered of these pathologies.
- Introduction : l'arthrose et les tendinopathies sont des pathologies dont la prévalence augmente, souvent résistantes aux traitements conservateurs et qui ont tendance à se chroniciser. Les biothérapies et plus particulièrement le plasma riche en plaquette (PRP) prennent une place importante dans la prise en charge de ces pathologies. Le but de cette étude est d'évaluer à moyen et long terme la douleur selon l'EVA et la capacité fonctionnelle dans les pathologies de l'appareil locomoteur, à la suite d'un protocole d'injection(s) de PRP. Méthode : étude quantitative transversale rétrospective, auprès des patients atteints d'une pathologie chronique résistante au traitement médico-rééducatif, ayant reçu une injection de PRP dans l'unité fonctionnelle de médecine du sport du centre hospitalier universitaire de La Réunion avec un suivi minimal de 18 mois. Résultats : 125 patients ont été inclus, divisés en 5 sous-groupes : 58 patients atteints de gonarthroses, 37 de tendinopathie d'Achilles (TA), 19 d'aponévrosite plantaire (AP), 6 de tendinopathie rotulienne (TR) et 5 de lésion ostéochondrale du dôme astragalien (LODA). Une diminution significative de l'EVA a été observé pour chaque pathologie (TA -4.70 ; AP -6.10 ; LODA -3.20 ; gonarthrose -3.72) sauf pour les TR. L'antalgie persiste à moyen et long terme (TA 19.33+-11.83 mois ; AP 18.52 +- 11.50 mois ; LODA 13+-13.29 mois ; gonarthrose 11.48 +-10.23 mois). Chaque sous-groupe présente une augmentation des capacités fonctionnelles évaluées par le nombre d'heure d'activité physique par semaine (h/sem) (TA +2.48 h/sem ; AP +4.3 h/sem ; LODA +4.6h/sem ; gonarthrose +3.28 h/sem) sauf pour les TR. Conclusion : notre étude est en faveur d'un effet antalgique à moyen et long terme dans la TA, la AP, la LODA et la gonarthrose et d'une augmentation des capacités fonctionnelles chez les patients atteints de ces pathologies.
